    Children born in Italy need to be registered in a Civil Registry Office in Poland (procedure), before submitting the application for a passport.

     

    A minor who is not more than 13 years old – receives a passport for 5 years.

     

    A minor who is more than 13 years old – receives a passport for 10 years.

     

    A minor who is more than 5 years old has to be present at the moment of submitting of the application for passport.

     

    An application form for a minor’s passport shall be submitted by both parents personally presenting the following documents:

     

    • previous passport if it was issued (or a passport of a parent if the child was registered in the parent’s passport) + photocopy,
    • if a minor does not have a PESEL number: original of a Polish birth certificate + photocopy,

     

     

    • a legibly filled out application form containing data of a minor together with a written consent of both parents for a Polish passport to be issued for a minor,

     

    • valid identity cards of both parents (for verification) + photocopies (Polish citizens present valid Polish passports or Polish identity cards, citizens of the European Union – passports or identity cards, citizens of other countries – passports)

     

    NOTE please that all fees shall be made in cash directly at the passport office.

     

    Moreover:

     

    • In case of a first passport:  an original of Polish birth certificate + photocopy,
    • An identity card, if a minor has not had yet a passport but she/he has an identity card (for verification) + photocopy,
    • If a minor does not have neither a Polish passport nor a Polish identity card since many years – a certificate of Polish citizenship or a decision of granting Polish citizenship shall be required + photocopy,
    • In case of applying for a passport because the surname or other data has been changed: a birth certificate,
    • In case of a loss of a currently held passport of identity card: we suggest to report the loss to a local police (so called “denuncia”) and obtain its confirmation + photocopy of such a confirmation; In addition it is necessary to fill out a statement regarding the circumstances of the loss (forms available atthe passport office),
    • Optionally a document confirming parents’ stay in Italy, e.g. an Italian ID card, a residence permit, a work contract, a student card or a pensioner card + photocopy

    In case of absence of one of the parents, the parent submitting an application form for a passport for the minor should present the following documents:

     

    • a written consent of the other parent confirmed by a public notary, a Polish consul or a relevant passport office in Poland (consent example)

     

    • In case of lack of the consent between parents or of lack of the possibility to get the consent of one of them: original decision of the competent family court containing the consent for a passport to be issued for a minor.

     

    • If the parental authority is granted solely to one of the parents: an original decision of the family court; in case of a foreign court decision - a certificate of its origin issued by a relevant court on the clear demand of the person involved (certificate form – Annex no II, p. 22) + photocopies,

     

    • If the child has only a mother: an original of a Polish birth certificate + photocopy,

     

    • In case of death of one of the parents: an original of death certificate + photocopy.

    In exceptional situations, when the minor does not have a PESEL numer yet, the consul can issue a first temporary passport with a validity up to 12 months.

     

    The following documents are required:

     

    • an application form containing child’s data together with a written consent of both parents for a Polish passport to be issued for a minor,
    • a foreign birth certificate + photocopy,
    • 1 photo (ICAO rules),
    • valid identity documents of both parents (for verification) + photocopies (Polish citizens present valid Polish passports or Polish identity cards, citizens of the European Union – passports or identity cards, citizens of other countries – passports).

    Children born in Italy shall be registered in a Civil Registry Office.
