    BIOMETRIC PASSPORTS FOR ADULTS (VALID FOR 10 YEARS)

     

    Those who got married abroad need to register first a foreign marriage certificate in a Civil Registry Office in Poland (procedure), before submitting the application for a passport. It’s not possible to issue a passport without prior registration.

     

    An application form for a passport shall be submitted personally with the following documents:

     

    • currently owned Polish document confirming the identity and Polish citizenship: a passport (even if it is no more valid) + photocopy and an identity card (for verification) + photocopy,
    • 1 photo (ICAO rules) or 2 photos in case of application for a temporary passport while waiting for a biometric,

     

    • legibly filled out passport application (available at the passport office).

     

    NOTE please that all fees shall be made in cash directly at the passport office.

     

    Moreover:

     

    • in case of a change of  surname or marital status: an original Polish marriage certificate (or a Polish marriage certificate with a note about the divorce and the change of surname) + photocopy, (those who got married abroad need to register first a foreign marriage certificate in a Civil Registry Office in Poland),

     

    • in case of the change of other data: original of a Polish birth certificate + photocopy,

     

    • in case of applying for a first passport: original of a Polish birth certificate + photocopy,

     

    • if a person does not have a PESEL number (Polish identification number): a Polish birth certificate (for those who are married also a Polish marriage certificate is required) + photocopies,
    • In case of a loss of a currently held passport of identity card: we suggest to report the loss to a local police (so called “denuncia”) and get its confirmation + photocopy of such a confirmation; in addition it is necessary to fill out a statement regarding  circumstances of the loss (forms available at the passport office),
    • If an adult has never had a Polish passport and/or identity card or these documents are not valid since  a long time: certificate of Polish citizenship or a decision of granting Polish citizenship + photocopies,
    • Optionally a document confirming a stay in Italy, e.g. an Italian ID card, a residence permit, a work contract, a student card or a pensioner card + photocopy.
